
The ACT or Acute Care transition Clinic was crated as a virtual follow-up clinic for patients that were seen in the hospital. There are many patients seen in the ER for significant illnesses that don't have a family doctor or in some jurisdictions, no prospect of finding a family doctor. The same can be said for patients who are discharged from the hospital with significant follow-up requirements. The ACT is a clinic to help these vulnerable patients.

The subclinic "watch" is the flagship clinic of the Clinomics clinic. Patients with little of no primary care are able to graduate from the ACT clinic, into a longer home monitoring program. Here patients with certain chronic disease conditions can be monitored by our healthcare team on a long term basis. Patients can input symptoms, perform symptom scales, record lab data, and input vital signs. Every time they are seen by a physician they are assigned a "C-score", which is a triage tool that helps determine how closely they need to be monitored by our team or if possibly they need to be seen by one of our specialist physicians. They also have ongoing access to the C-clinic, a virtual clinic designed to address acute exacerbations in their condition. If patients acquire a reliable primary care provider, they can graduate out of watch in to their family doctors care.

Every time a patient is reviewed by a physician, the patient is assigned a score from 1-6. Clinomics uses an algorithm to adjust the C-score based on a number of factors including physician judgement, symptom scores, lab data, and vital sign data. It acts a a triage tool to help us determine how robust the follow-up needs to be for that individual patient.





The Clinomis clinic has partnered with the groundbreaking remote patient monitoring (RPM) company Persona Informatics. This gives Clinomics the ability to seamlessly integrate multiple different types of RPM devices into our medical data stream. This gives us the accurate data we need to perform our predictive analytics on patient outcomes and adjust our follow-up and clinical interventions accordingly.




The Clinomics clinic strives to encourage a hybrid model of care where patients can be seen virtually for their convenience but also have the option for an in person assessment should the need arise. It is especially helpful to be seen in person for physical examinations, remote patient monitoring (RPM) onboarding and support, and onboarding for the Clinomics app. Our clinic in Scottsdale, Arizona is a state of the art modern clinic in the heart of North Scottsdale. We offer our patients a beautiful clinic experience as well as a large mobility and physical assessment space within our Zerone Performance Lab.

Patients access all the features of the Clinomics clinic via the Clinomics app. Patients are eligible to join after participating in the Acute Care Transition program after a hospital based interaction (ER or hospital discharge). Any healthcare provider can refer you to the ACT program.
